Suddenly loss permission to access /wp-admin/ on all websites.

Was just reworking one of my WordPress sites and suddenly got the following error in my web browser.

I checked my other WordPress install on my DA server and that too has this error.
Not touched any settings for a few days and it has been working fine since install.

Any ideas what could cause this?

Getting a 403 in the error log.
 
Here is the log. Just noticed it is trying to use private_html instead of public_html...

Bad news.
How did you get hacked? Was it a leak theme or plugin? Or a WP installation not up to date?
Might be interesting to others to secure their WP installations.
