Email threshold hit but SMTP Log completely empty

Does anyone have any idea how the SMTP Log feature filters the In ⇄ Out exim log? We had an email threshold hit notification but the Out log for this email account is completely empty. When doing a manual exigrep it shows all the outgoing emails. Could it be the Out log filters on the from address (F=<...) only in stead of the user that's actually logged into the SMTP server (A=login:....)?
