feedback.php not working

I've used a free php script from http://www.thesitewizard.com/wizards/feedbackform.shtml

Did everything they said BUT it's not working.
The weird thing is that mail arrives even thought I get a 404 when trying to send !

Anyone knwo what I'm doing wrong?

I fixed it myself - I'd made a typo in the name of the 'thank you' page ......... :~(
