Lot connection failures

I was just looking in my phpmyadmin to the server stats and saw this:

Connections  	ø per hour  	%
max. concurrent connections 	12 	--- 	---
Failed attempts 	75 k 	578.53 	9.44%
Aborted 	2 	0.00 k 	0.00%
Total 	792 k 	6,130.61 	100.00%

The mysql server was running already for 5 days. I think this 10% of connection failures is a bit high, how can I prevent this failures, or if I cant prevent that, what is the reason the connections failed?

The server is a owned box with newest DA and mysql 5.0.45.
