SpamAssassin user settings fail

One of our clients has discovered that whether it's set in the user-level SpamAssassin settings or not, the *****SPAM***** is always put into the subject. It appears that no changes are taking place even when the settings are changed.

Latest SpamAssassin (updated today), isntalled by spamd.sh. Well tested.

Has anyone else seen this or similar problem?

Check the headers of the message to see what's being flagged.
