So how do you get from no reference to it at all, to adding the false qualifier?

Two facts here, from the guy who wrote both the current official exim.conf file, and my own:

1) It's worked for lots of us for a long time exactly as documented.

2) Including it without the qualifier sets it, including it with the qualifier may or may not; I've not read the exim sourcecode to see; if it stops reading at the first whitespace it'll set it; if it ignores the spaces and parses it as if it had no spaces it, will unset it.
