how do we customize nginx.conf when running nginx as proxy to apache?

theres a lot of settings that can be done when nginx proxied to apache yet I cannot see a manner to add them.
direct edit of the user/nginx.conf file leaves site down.
theres no way for me to edit any nginx settings that work when I am running proxy.
and when running as proxy apache needs to be compiled with mod_remoteip

I've seen this but it makes no sense to me
