how to make a 'response form' on a html website?

I did try to make a 'response form' on my html website. First I tried to do it with WordPress, but that didn't work out for me.
Now I have a PHP code, but I end up with a 404 message, like the one here in the appendix.

So this is my question:
How to make a response form without using an outside company to use my emailaddress and send me messages from my website?

Please ask on a computer forum or someting (try nationaalcomputerforum.nl for example). This forum is for Directadmin (licenseholders) howto's and support , not for website support.
Do you have a DA license? Or do you only use DA as hosting customer?
 
I know Richard G, I'm a licenseholder from Directadmin and I've try to work it out via Directadmin, but there wasn't any solution. That's why I add my question here.
 
Oke sorry then, but it's still the same, the DA forum is not intended for website support, you can better use a PC forum for that, because those are really no DA questions but website questions, this forum is not intended for those questions. NCF is a good forum.
Or otherwise use the off-topic section here. ;)

P.s. it seems the Formmail.pl file is not found, start checking that. ;)
