Problem with MySQL Management

MySQL Management has well done work
after create/delete/create/delete/create/delete DBs processes
I cant't creat new DB
1 of DB is good work now
if i click to
Create new Database
I see
Error: document not found
My dedicated suppot say. that need reinstall all DApanel
But, I can' OFFLINE my webserver for 3 hour
how I can repair or reinstall only MySQL Management ?

Check for errors in /var/log/messages.
