Server Blocked IP

HI there

My server has just blocked an IP lets say aaa.aa.aaa.aaa !

I have done:

apf -f
iptables -F

And none of those have worked!

I have disabled the firewall has well and no luck.

Any clue on whats about??

What OS?
Waht exactly blocked it? (Firewall/application/webserver?)

Without this... we'll be unable to help ...
